Supplementary Resource Index + Request Form

This is a document of supplemental resource materials-- that is, materials approved after the initial curriculum revision for each academic discipline.

This is the Supplementary Resource Request Form-- that is, requests from CFSD educators that a material be reviewed and approved for use during instruction. Fill out this form to request that the Governing Board approve any resource which:

• you plan to use regularly (every year)
• contains content and/or
• is a technology resource (this needs CTI team approval)
